Made-up names and activity. Replace every field with what you actually did and the details you have.

Download the blank weekly worksheet to record your own activities, then compare it with the completed fictional week below. Each entry shows the activity date, job and employer details, contact method, and result found on the Texas Workforce Commission (TWC) log. Enter only work-search actions you actually completed, and use the number of activities assigned to you.12

What to put in a Texas work-search log

TWC's Work Search Activity Log has spaces for the benefit week and the number of required searches. For each activity it asks for the date, what you did, the type of job, employer or service, address or website, city and state, phone, contact person and method, and result.2 The fillable worksheet above follows these fields so you can prepare your own weekly record.

TWC says you may keep your own log if it contains the same information as the official form, and you should be ready to submit a log if asked.1 For the agency's form and current instructions, use the official TWC links below.

Read the completed example week

The four entries for September 20–26, 2026 are fictional. Open each activity above to inspect its fields, or download the completed PDF to see them together. The assigned number of required activities is deliberately blank: enter the number TWC gave you for your own benefit week.

The example includes online applications, an emailed application, and a phone interview. Juniper Desk's online application shows how to record a submitted application when the employer has not replied. The sample website and email use example.com and are not real vacancies.

A submitted application and an employer reply are separate events. Record the submission, then state the response status you actually know.

How to record an application with no response

For an online application, record the date you submitted it, the job and employer, the website or contact route, and the result. If you have no response, write 'application submitted; no reply yet' rather than guessing what the employer decided.

Keep a confirmation email, application number or account history with your own records when one exists. Update your log if a reply arrives. Check TWC's instructions if you are unsure how to enter a detail on its form.2

  1. Enter the actual date of submission, not the date you planned to apply.
  2. Copy the employer, role and website details from the real vacancy or application account.
  3. Record the contact route and the result you know today. Update the record if a reply arrives.
  4. Keep the original application material separately and follow any TWC request for a log through its official channels.

Check your own weekly requirement

Your assigned weekly requirement belongs in the box at the top of the log. TWC sets this for your claim; the four activities in our sample are simply an illustration, not a required number. Check your own notice or TWC account for your number and benefit week.13

Record the activities you actually completed during that week and follow TWC's current instructions if it requests your log.13

Use a personal organizer without losing the form details

ApplyBrio's job-search log can help you remember dates, roles, outcomes, evidence references and next steps across applications. You can export a CSV or print those notes through your browser.

For a Texas work-search record, use the fillable weekly worksheet or TWC's official form so the contact details from its form are present. Compare any personal record with TWC's current form before using it for a claim.12
